If you are a member of the Class as a consumer, you may file a claim for a share of the Settlement Fund. You must complete a Claim Form and mail it to the Notice and Claims Administrator so that it is received no later than August 5, 2026, or you can submit your claim online no later than August 5, 2026.

Third-Party Payors
Third-party payor (“TPP”) class member are entities that purchased, paid, and/or provided reimbursement for a metformin-containing drug for consumption by their members, employees, insureds, participants, or beneficiaries. They include entities such as health insurance companies and self-insured health and welfare plans.
If you are a member of the Class as a TPP, you may file a claim for a share of the Settlement Fund. You must complete a Claim Form and mail it to the Claims Administrator so that is received no later than August 5, 2026 or you can submit your claim online no later than August 5, 2026.
Transaction data is mandatory for claims of $100,000 or more, although the Claims Administrator may also require transaction data for claims of less than $100,000, so keep related transaction data and any other documentation supporting your claim in case the Claims Administrator requests it later. If your claim is for less than $100,000, you should still provide the transaction data with your claim submission if you can.
